SB - Choosing different options after selecting a subscription plan

we want to sell a supplement subscription. customers should be able to choose between three main options: one time purchase, monthly sub and double monthly (for pairs). once they chose one option (let's say monthly sub), they should be able to choose their gernde male/female so we know which specific supplement line to ship to them. i'll attach a screenshot from seal subscription app which supports this feautre.

the last option shown is the double sub, which allows to configure the sub as in male + female / male + male / female + female
